1.Access the NDIS: •If you or the individual you are supporting is not already an NDIS participant, you need to go through the NDIS access process to determine eligibility. You can apply for access online or contact the NDIS for assistance.
2.Assessment and Planning: •Once eligible, you will go through an assessment process to determine the level of support required. Based on this assessment, an NDIS plan will be developed.
3.Include Support Coordination in the Plan: •During the planning meeting with the National Disability Insurance Agency (NDIA), you should discuss the need for Support Coordination services. If it is determined that Support Coordination is necessary to help implement and manage the plan, it will be included as part of the plan.
4.Budget Allocation: •The NDIA will allocate funds specifically for Support Coordination in the NDIS plan. The budget allocation will depend on the individual’s needs and goals.
5.Selecting a Support Coordinator: •Once the plan is approved, you can choose a registered Support Coordinator or organization to provide the service. You should consider factors such as their expertise, experience, and compatibility with the individual’s needs.
6.Service Agreement: •Work with the chosen Support Coordinator to establish a service agreement that outlines the scope of services to be provided, the fees, and other relevant terms and conditions.
7.Claiming for Services: •The Support Coordinator will claim payments for their services directly from the allocated Support Coordination budget in the NDIS plan.
8.Monitoring and Review: •The individual’s NDIS plan will be reviewed periodically (usually annually), and any necessary adjustments, including changes to the Support Coordination budget, can be made based on changes in needs or circumstances.
It’s important to note that Support Coordination funds are allocated specifically for the purpose of helping the individual implement and manage their NDIS plan. The NDIS plan is developed based on the individual’s goals and needs, and Support Coordination is intended to assist in achieving those goals. It’s recommended to work closely with the individual, their family, and any relevant professionals to ensure that Support Coordination services are tailored to meet their specific requirements and aspirations.
